1.You were clingy
When she gave you her phone number, and you danced around and got all excited, you lost it there buddy. If it does not look natural then you've gotta build up your inner game.
2.She is not in the moment anymore
SYou are not in her mind. She cannot remember what your face looks like. You didnt make that much of an impression. These are all possibilities. This is why I recommend planning something while you are in the moment. This way, if she flakes on you, there is a recovery period for you.
3. She heard something about you
Girls talk...and talk... and talk.. and talk.
We cannot blame them though. It is a defense mechanism that protects them from getting hurt. If you have built a bad reputation, there is a good chance that it could have contributed. This is especially particular if you are quite popular in your circle or area.
4. She actually had something to do
Girls are beautiful human beings guys. They are human beings. If you objectify them, then there is a good chance you will not see that they in fact, have things that come up out of no where.
5. She really was not into at all, and she felt bad
Sad, but true. It happens. Some girls cannot be direct. Move on and do not let it bother you. There are many out there.
